Patricia St. Martin, RN, WCC
As Care Transitions Coordinator (CTC), Patricia St. Martin works with patients, families and referral sources to set up care plans to transition patients from health care facilities back to their homes. She meets with patients and their family members to discuss discharge plans and goals, and she recommends adaptive equipment when necessary and appropriate services to ensure a successful transition home.
Prior to her role as CTC, Patricia served as a Case Manager for Natick Visiting Nurse Association. Previously, she was a per diem Ambulatory Nurse at Tufts Medical Center. She worked in several specialty clinics, including those focusing on wounds, pulmonary issues, infectious disease and pediatrics.
An associate board member of the Natick (Mass.) Council on Aging, Patricia’s expertise lies in gerontology and wound care, and she holds a Wound Care Certification from the Wound Care Institute. She holds a Bachelor of Science in Nursing from St. Anselm College.
She and her husband have two children and live in Natick.
